Outline of Coverage:
Office Visits: After the $15 copay, the policy pays 100% of office visit and additional charges incurred while in the office and billed by the doctor, up to the aggregate $2,000 outpatient annual maximum.
Other Outpatient Expense: Subject to the $200 calendar year deductible, the policy pays 80% of eligible expense up to the aggregate $2,000 outpatient annual maximum.
Prescriptions: Subject to the $200 calendar year deductible, the policy pays 80% of prescriptions up to the aggregate $2,000 outpatient annual maximum.
Inpatient Hospital (confinement exceeding 24 hours): Subject to the $200 calendar year deductible, the policy pays $200 a day while confined ($400 if in ICU or CCU) and 80% of miscellaneous hospital expense up to the annual miscellaneous hospital maximum. Aggregate inpatient annual maximum is $4,000.
Surgeon: Subject to the $200 calendar year deductible, the policy pays 80% of surgeon's charges up to the aggregate $4,000 inpatient annual maximum and/or the aggregate $2,000 outpatient annual maximum.
Anesthesiologist: Subject to the $200 calendar year deductible, the policy pays 80% of anesthesiologist's charges up to the aggregate $4,000 inpatient annual maximum and/or the aggregate $2,000 outpatient annual maximum.
Pregnancy: Covered as any other illness after policy is in force 10 months.
Well Care: Coverage for sickness, accident or well care subject to the aggregate outpatient $2,000 maximum.
No Networks: Freedom to visit any doctor with no penalties.
Pre-Existing Conditions: Covered after 6 month waiting period.
